WHAT IS AN INDEPENDENT CASE MANGER? & WHAT WE DO?
Definition: Case Management is a method which aims for the continuity of services and quality care among specifically identified clients in a context of efficient, effective and humane resource management.
What We Do For You:
Asses the individual’s level of injury and disability
Identify the injured individual’s support network
Identify the expectation of the individual’s family
Coordinate all tests, examinations, consultations and care according to the condition of the patient and clinical pathway plan
Advocate for the injured individual’s welfare
Serve as liaison between the injured individual, their families, healthcare providers, insurance providers (adjusters) and legal team
Educate the injured individual and their families on diagnoses, symptoms to be aware of medications, procedures, and available services.
Work with the hospital’s discharge planners / social workers to arrange for and manage durable medical equipment (DME) needs of the injured individual
Schedule all medical appointments
Arrange for medical / non-medical transportation for the injured individual if necessary
Accompany the individual to all medical appointments to ensure proper treatment and care
Oversee the follow-up of consultations and that they are linked to other services
Eliminate task and intervention duplication that complicates the recovery process
Oversee the evaluation and satisfaction of client needs as well as the achievement of anticipated clinical outcomes and recovery plan goals
